Scotland Aero Services, Inc. operates 2 DZ's at MEB. DZ1 exclusively for the Golden Knights. DZ 2 is reserved for SPECIAL OPS units. THE FEE/FUEL PURCHASE WAIVER REQUIREMENTS APPLY TO BOTH DROP ZONES.

(Req'd) Usage fees are for each day. Usage fees are waived with fuel purchase. If aircraft commander does not purchase fuel as originally planned (150 gallons minimum), the fee will be reinstated AND WILL BE CHARGED TO YOUR IMPACT CREDIT CARD. NOTE: 48 HOURS CANCELLATION NOTICE REQUIRED OR UNIT WILL NOT BE CHARGED THE $500/DAY DZ USAGE FEE. When multiple aircraft are being used for the same unit, we ask that the above POC encourage the aircraft commanders to use our DOD contract for their fuel needs.

Note: Writing your name above denotes that this entire authorization form has been read and will be strictly adhered to unless exceptions have been granted prior to the jump mission by SCOTLAND AERO SERVICES, INC.

FAILURE TO DECONFLICT WILL RESULT IN DZ AUTHORIZATION DENIAL! If another unit has been given prior authorization the 2nd requesting unit is required to deconflict with the initially authorized unit.

The POC and phone number for the initial unit will be provided and at his/her discretion will allow the co-use of the dropzone.

Note: Authorization for both DZ's use at M.E.B. is valid only if an authorization number has been assigned and this form has been signed by the owner/manager of the FBO.

DZ 1 (GOLDEN KNIGHTS) ONLY A REPRESENTATIVE OF THE GK'S CAN AUTHORIZE THE USE OF THEIR TRAINING FACILITY AND CO-USE OF DZ 1. HOWEVER, THIS FORM MUST BE USED SO THAT SCOTLAND AERO SERVICES, INC CAN RESCHEDULE YOUR TRAINING MISSION FOR THE DATE(S) REQUESTED.

SAFETY REQUIREMENTS! All below are required and are critical to safety of flight!

a: Mon-Fri, Prior to the mission date and each morning of the mission, the DZSO for #2 DZ shall contact in person and coordinate with the DZSO for DZ #1 and DZ #2 at the Golden Knights training center. Common DZSO Frequency between DZ #1 and DZ #2 shall be 123.15 (SGT. John Aughtran C-190 670 0018, Team Cell: 270 304 9217, W-910 844 7065

DZSO for both DZ #1 and DZ #2 shall use a 2nd radio to monitor Unicom Frequency 122.8 During multiple operations, aircraft commanders shall coordinate among themselves their Climb positions, Jump positions, Altitude, Exit time, and any other pertinent information to ensure the SAFETY of their crews, jumpers, aircraft, and ground personnel at all times.

DZ # 2 HOUSING REQUIREMENTS: a: No vehicles allowed on Drop Zone (Military or Civilian) unless there is an emergency. There is ample parking on the concrete pads near the shelter. b: Police your trash items and place in trash container provided at the end of each day's mission. c: Close and lock gate upon leaving the drop zone.
